Challenges

Challenges of a Patient Living with a Nephrostomy Tube

Nephrostomy Tube Dislodgement

  • The tube can unintentionally be pulled out or kinked.
  • Occlusions can lead to hydronephrosis, pain, and infection.

Tube Blockage

  • Can occur due to blood clots, debris, or encrustation within the tube.
  • May lead to flank pain, fever, or hydronephrosis.

Discomfort and Pain

  • Minor movements or pressure on the area of tube insertion may cause pain.
  • Discomfort while sleeping, getting dressed or during daily activities.

Lifestyle Limitations

  • Restricted mobility, bathing challenges, clothing adaptation.
  • Impact on sexual activity, exercise and social participation.

Emotional and Psychological Burden

  • Due to the risk of unintentional removal of the nephrostomy tube, patients frequently constrain their physical movements and limit routine activities. This caution often undermines their confidence in performing day‑to‑day tasks and engaging socially.
  • Anxiety, depression and body image concerns are common.
  • Lifestyle modifications: bathing, clothing, and activity restrictions.
  • Body image issues, anxiety, or depression.
  • Fear of complications or accidental dislodgement.

Urine Leakage Around Tube

  • Often results from poor placement or clogging, or due to poor fit or blockage.
  • May cause skin irritation or breakdown, resulting in skin maceration, or lead to infection.

Infection Risk

  • Entry point through the skin increases risk of infection, one of the most common complications.
  • Bacteria can enter through the skin around the tube or from the drainage bag.
  • Signs include fever, chills, redness, or pus at the insertion site.

Ongoing Maintenance Needs & Burden

  • Requires regular tube flushing, and bag emptying, dressing changes, and proper handling of the drainage system.
  • Patients often need caregiver support or nurse visits.
  • Risk of mismanagement if patient or caregiver is untrained.
